Screen Reader Survey Instructions
We are conducting a survey on screen reader use to help us provide the best training possible.
Please use one of the following links to retrieve the survey as a Microsoft Word document or a plain text file. You will be prompted to save the file or open it. We recommend that you save the file you want to your computer so you can fill it out.
You have a number of choices to get your survey back to us.
1. You can attach your completed survey in an email to: ctcassist@guildfortheblind.org
2. You may print your survey and mail it to Guild for the Blind. 180 N. Michigan Avenue, Suite 1700, Chicago, IL 60601-7463
3. You can call the Guild on Wednesdays or Thursdays and ask for the volunteer who is conducting the screen reader survey at: (312)236-8569
