SCREEN READER USAGE SURVEY

Please answer the following questions about your screen reader experiences.  Your feedback will help with future program development.

1. Do you use screen reading software such as JAWS, Window Eyes, Hal or some other type?  If so, please specify.  If you know which version you are using, please indicate that as well.

2. How did you learn to use your screen reader (e.g. formal training, self-taught, other)?

3. Were you given a choice about which screen reader you could use?

4. On a scale of 1 to 5, with 5 being extremely important, how important is it for you to have a choice which screen reader you use?

5. Do you consider yourself to be a beginner, intermediate or advanced user of your screen reader?

6. What tasks do you most often do on the computer (e.g. email, using the Internet, composing documents, etc.)? 
7. What do you most like about your screen reader? 

8. What do you like least about your screen reader?

9. Do you experience particular problems or issues with your screen reader?  If so, what are they?
